GATHER INFO
For each health plan offered by your employer(s), you’ll need your:
annual premiums,
self, self + spouse, self + kids, all (as applicable)
annual employer contributions to an HSA or HRA,
self, self + spouse, self + kids, all (as applicable)
an uploadable copy of the summary of benefits & coverage (i.e., SBC).

SUBMIT FORM AND SCHEDULE AN APPT
Click the appropriate button below to input your info into an intake form. You must log into Google to access the form. The form will end with a link to schedule and pay for an appointment.

We’ll meet virtually to help you choose the right health plan(s) and answer other health finance questions!
CANCELLATION
& RESCHEDULE POLICY
Cancellation Policy
If you cancel at least 24 hours in advance, you will receive a full refund. If you cancel less than 24 hours in advance, you will only be refunded 50% of the appointment charge.
Reschedule Policy
If you reschedule an appointment at least 24 hours in advance, you will not incur an additional charge. If you reschedule an appointment less than 24 hours in advance, you will only be refunded 50% of the original appointment charge and be subject to the full cost of a reschedule.
Emergencies and unforeseen circumstances will be considered on a case-by-case basis.
